GROWTH HORMONE AXIS / COMPARISON

Signals, feedback, and measured responses

A side-by-side view of mechanisms, evidence types, continued-exposure observations, and unresolved attenuation questions.

The short version

The compounds in this review share a growth hormone (GH) pathway, but the studies ask different questions. Sermorelin, CJC-1295, and Tesamorelin act through the growth hormone-releasing hormone (GHRH) receptor. Ipamorelin adds a ghrelin-receptor route to the pairing. A receptor is a cell's signal receiver; using a different receiver can change the response without proving a better outcome [5][6][8].

Negative feedback means the hormone system applies brakes to further release. Response attenuation means a measured response becomes smaller with continued exposure. They are related questions, rather than interchangeable explanations [8][11].

The table below separates physiological reasoning from observed findings. CJC-1295 provides hormone-persistence and pulse data. Sermorelin provides pediatric growth evidence and a feedback rationale. Tesamorelin provides continued-exposure body-fat outcomes. The fixed combination lacks its own controlled clinical trial in the supplied literature [3][7][12][15][19]. None of these sources supplies a direct attenuation ranking across all the members.

Mechanism is the starting point

The GHRH analogues act upstream of GH release. Somatostatin provides inhibitory signaling, and IGF-1 participates in feedback through the endocrine system [8][11]. Ipamorelin acts through a different receptor. The combination rationale is therefore about complementary stimulation, rather than replacing the feedback system with a simple on-off switch [5][6][7].

The exposure profile is another distinction. Albumin binding extends the DAC form of CJC-1295, whereas the commonly named no-DAC form lacks that feature [3][4]. The pairing's source literature does not establish a clinical exposure-response profile for the fixed blend. A shared class label is not enough to assign comparable duration, magnitude, or risk.

Continued exposure answers different questions

Teichman and colleagues found that IGF-1 remained above baseline for up to 28 days following repeated CJC-1295 administration [3]. That documents persistence of a hormone response. It does not establish whether every later administration produced the same incremental rise, or whether clinical benefits persisted.

Ionescu and Frohman found an unchanged pulse frequency and magnitude with CJC-1295 at the sampled interval, even while basal GH increased [15]. That addresses secretion pattern under prolonged stimulation. It is not a lifetime assessment of pituitary sensitivity.

Falutz and colleagues reported that Tesamorelin's visceral-fat reduction remained at 18% over 52 weeks, while fat reaccumulated after discontinuation [19]. That addresses an outcome during continued exposure and after stopping. Reaccumulation following withdrawal is not the same as a diminishing response while the compound is still administered.

The Sermorelin safety summary raises possible blunting under constant stimulation, but the associated primary study is absent from the retained reference list. This comparison leaves that as an evidence gap and supplies no invented duration or magnitude. Feedback physiology remains supported by the review and editorial [8][11].

Evidence maturity depends on the question

Tesamorelin's controlled trials and pooled analysis address visceral and liver fat in adults with HIV-associated lipodystrophy [1][17][19]. Sermorelin's direct outcome study here concerns growth in GH-deficient children [12]. The adult cognition trial carried in its source set used Tesamorelin, so its findings cannot be attributed to Sermorelin [9].

CJC-1295's human studies emphasize hormone dynamics and candidate biomarkers [3][14][15]. The pairing relies on component results and related receptor-synergy research [5][6][7]. A larger result in a cell-signaling assay does not establish a larger human benefit. Evidence maturity therefore belongs to an endpoint and population, rather than to a universal league table of compounds.

Safety does not follow from preserved feedback

The secretagogue review identifies blood-glucose and insulin-sensitivity concerns, with long-term cancer and mortality data still needed [2]. Persistent IGF-1 elevation and theoretical proliferation questions are relevant to the GH axis [3][8]. Feedback, pulse preservation, or a favorable short study cannot settle those longer-term questions.

Regulatory context also differs. Tesamorelin has a specific approved HIV-associated indication [16]. Sermorelin's historical pediatric approval does not establish current adult wellness claims. CJC-1295 and Ipamorelin have no approved human indication in the supplied record. All the members are identified there as prohibited in sport. Research-grade identity and sterility remain separate from a molecule's reported mechanism.

The comparison is thus an index to the literature, not a selection guide for human use. Each compound dossier preserves the study setting, and the references supply the original source details for the claims that can be checked within this edition.
